I won't say this is a must but it has great italian food and it is very centric. This restaurant brand has been in Puerto Vallarta for many years.

A really nice Italian restaurant in Old Town. You can't go wrong with their dishes and pizza.

Great patio space and air conditioned dining room. Food is decent, not the best Italian in town but sometimes you need the AC. Service is great.
